Strengthening
NGO Capacity to Improve Maternal and Child Health Status
through Implementation of Life Cycle Approach (LCA) in
Jharkhand

The Population Foundation
of India and Sir Dorabji Tata Trust are together supporting
a project with Child in Need Institute (CINI), Kolkata to
strengthen the NGO capacity in the state of Jharkhand to
implement Reproductive and Child Health Programmes through
Life Cycle Approach and to bring about improvement in the
RCH status. The project is implemented in Churchu Block of
Hazaribagh district in Jharkhand with Nav Bharat Jagruti
Kendra as the implementing agency in the field. The main
objectives of the project are to: |

Strengthen
the capacity of four partner NGOs to provide integrated RCH-HIV
services by using life cycle approach in four divisions of
Jharkhand. |

Develop and implement the life
cycle based community level interventions to improve safe motherhood,
child survival and growth, achievement of fertility goals,
male participation, adolescents’ informed choices about
health and health delivery in one block setting (Churchu block
of Hazaribagh district) through one partner NGO. |

Document and disseminate the
experience gained through the project to other NGOs in Jharkhand
and the State Government. |

This is a five year project started during the year under report - the first
phase of six months was on strengthening capacity for developing intervention
package and conducting baseline study. A six-day workshop was organized by CINI
on survey methodology for the Nav Bharat Jagriti Kendra (NBJK) staff, field staff
and a few external surveyors for conducting the baseline survey. The survey revealed
a high level of awareness on ANC, TT, IFA, sterilization and childhood immunization,
but low awareness on danger signs of pregnancy and child birth, five cleans,
importance of institutional deliveries, newborn care, signs of HIV/RTI/STI and
advantage of condom use.

| The second phase of 48 months
is on and would be focusing on strengthening capacity for
programme implementation with a desired level of quality
of care and capacity development of other NGOs. |

The project was officially launched in August
2004 and 20 female workers and 5 male supervisors were recruited
and trained on RCH issues at a six-day workshop. Based on
the findings of the baseline survey, the Detailed Implementation
Plan (DIP) was developed and the budget was reallocated to
provide more attention to the underserved areas. Sixty eight
Village Health Committees (VHCs) have been formed by NBJK
staff so far and oriented on their roles; 30 Sakhiyas have
been selected and oriented on RCH issues, communication strategies
and their role and responsibilities at a three-day training
programme. They are working in close coordination with the
Health Workers of the project. |

| Field visits were made by the project staff
to observe the functioning of VHCs. For experience sharing,
members of VHC visited a VHC located at another village. A
two-day training programme on post natal care, breast feeding
and field testing of community tools was organized for the
project staff. Community monitoring tools for Sakhiyas to track
the services of ANC/PNC and immunization have been developed. |

| Training was given to health workers on how
to use a pregnancy-color test kit for identification of early
pregnancy (within 12 weeks). Health camps are organized for
ante-natal check-up and immunization is provided in 11 sub-center
areas where there are no government facilities. Private institutional
linkages are also built for high-risk cases as government facilities
are poor. |

A Behaviour Change Communication (BCC) matrix
was developed, modelled on the existing ideal practices,
strategies to change and mode of reaching out to people.
Based on that, a draft module has been prepared. The IEC
materials have been made available to the Sakhiyas, and Nukkad
Nataks and wall writings have been carried out. Male groups
were formed for male involvement on RCH issues and contraception,
and group formations of pregnant and lactating mothers were
made for discussion on RCH issues including government schemes
by the project staff and the ANMs. Management Information
Systems (MIS) formats have also been prepared. |
